How to Add a MAR (Medication Administration Record)

How do I add a MAR to a patient record?

If a product has been designated as needing a Medication Administrator Record on your patient record, then the section to add a MAR will automatically appear on the episode once the prescription has been requested whether that is from the pharmacy or a CDM.  

Any other prescriptions issued that do not require a MAR will also appear in a separate section of Medication.

To enter the Medication Administration Record, click on the X next to the drug that you gave to the patient. The MAR window will open for you to track the medication.

The Medication Section shares its deficiency X on the header with both MARs and missing Vaccine Certificates. A missing MAR will override the vaccine certificate, but both must be filled out to clear the deficiency on the Medication section.

How do I know if my quantities are correct?

VetView's MAR will rely on adding up the total quantity administered through all methods to match the dispensed quantity.  If ten tablets were issued, then ten tablets must be accounted for.  If 30 ml of liquid were dispensed, then 30 ml must be accounted for.  Used Methods include being dispensed to the patient, Returned (to the pharmacy), Wasted (in the approved method or receptacles used by your hospital), and Sent Home with the client (for patients who are being discharged earlier than anticipated.)

 

Why do the MARs ask for my password?

In order to enter a MAR, the password of the user who Dispensed or Administered the drug must be entered.  For Wastes, the waste method must be entered, and another username entered for the product witness. 

After the MAR has been entered and all of the dispensed quantity accounted for, the flag on the Medication section will turn to green, indicating that the MAR is completed.

VetView has added several ways to find your patients with incomplete medication records besides looking directly at the Episode and the patient record.

Is there a quick look up for only my incomplete MARs?

From the Home page, you can add a widget that will display any patients that have an incomplete MAR to which you have been assigned. This is My Incomplete Medication Administration Records.

Select the record you want to complete, and click on the Edit button.  The MAR will appear directly on the Home page, and you can complete the record from there.

Can I find incomplete MARs on the patient list for my unit?

An incomplete MAR is considered a medical record deficiency.  As such, it will appear in the Deficiency column (as Medication) on any patient visible on the Patient List when it has been added to an episode but not yet completed.

As an administrator, is there a central place to find all of the incomplete MARs for my pharmacy?

For Pharmacists and Supervisors with access to the Pharmacy Administration Search screen, it can be used to find all incomplete MARs for a patient, a specific employee, or through the entire hospital in a date range.  This screen is also used to build out the DEA reports for drugs dispensed.  Please see Pharmacy Administration Search screen for more details.

Medication Administration Record

A form documenting the usage of medication while a patient is in the hospital. This form is used to track the quantity of the medicine, the time it was dispensed, and if there is any amount left over, how it was handled. This form is required by state and federal law for controlled substances.
