How to Create a Client Contact on the Communication Log

The Communication Log is a small panel on the right hand side of the Patient Record that allows you to record any time you spoke with a patient's contact through any method.  Documentation that is sent via fax or email is generally handled via the Communication Worklist at a central location to allow the documentation to be reviewed ahead of time.  The Communication Log is intended for students, staff, and clinicians to note when they spoke to a client more informally, such as over the phone or via email conversation.  Formal communications from the Communication Worklist will also appear on this log automatically.

How do I add a new entry to the Communication Log?

Click on the New Entry log.  A series of fields will appear.

The date and time will be added automatically.  Select the method of communication, and check the list of available contacts in the drop down menu.

Add the message that was given, and any other internal notes that someone else in the clinical care team might want to know (client isn't available by phone before 5PM, for example.)

How do I add a new contact?

If the person you spoke with isn't available on the list, you can add a new contact to maintain accuracy in the log.

Click on the Add New Contact button to start this process.

The standard New Client dialog box will open.

Fill out the New Client Contact form like you would any other place, and then click on the Save button to add them to the Patient Contacts.

The newly added Client Contact will automatically appear in the Contact section of the communication log.

Click on the Save button to complete adding the communication to the log.

The client will now be listed under the Patient Contacts tab, and you can add any additional details to them after you have recorded the communication, such as indicating they are the primary care clinic for the patient in this example.
