Changes to the Web Portal Account Management Screen

Objective: Allow the user to assign a Hospital Web Portal Account as allowed to submit referrals.

Add a flag for Submit Referrals to the Hospital Web Account Portal Add/Edit Window

This should be modeled on the existing option for Labs Web Account Portal.

Only Client Account Roles of RDVM or Clinic should have this option; owners cannot submit referrals.

Display this flag on the Linked Hospital Client Accounts window

Add a column for Submit Referrals Y/N on this window.

This flag will control whether a logged in user can see the Referrals tab on the Hospital Portal when the hospital has enabled it for the entire site.
